A reaction at equilibrium has forward and reverse rates that are identical (so the net concentrations of reactants and products do not change with time).
A reversible reaction that is not at equilibrium has different rates of the forward and reverse reactions. Therefore, the concentrations of reactants and products will change in a way that approaches the equilibrium values.
