If an enzyme is not in its optimal conditions, it doesn't work as well. If this occurs, then tissues can be damaged if the enzyme affected can't break down the substrate fast enough.

For example, if the enzyme catalase, which breaks down hydrogen peroxide,is not at a good functioning range, the body can't break down the hydrogen peroxide fast enough so there's a buildup of the toxic substance which can lead to tissue damage, or worst case death.
