Chromosomes are Rod like structures present inside the nucleus which is first seen clearly during the Metaphase of cell cycle.

During Resting phase of cell , due to high water content in the nucleus , these chromosomes uncoil and appear as fine thread like structures called Chromatin.

Basically chromosomes are formed by condensation ( loosing water ) from chromatin.
