What should we do when we come across an answer that is entirely wrong?

I've been going through editing some questions and checking others that have been flagged for inaccuracy, and I'm wondering whether I should rewrite/reword the answer that is written by someone else already, or just add another answer completely.

Answer:

Flag it as "Check for Inaccuracy" (Under suggested improvements) and write a comment as to why you believe the answer is wrong.

Explanation:

Certainly if you have the time (and inclination) writing a new answer is highly desirable.

Modifying an existing answer should (in my opinion) only be done when the changes are relatively minor (e.g. corrections in arithmetic, formatting, etc.)
