Different layers of Earth's atmosphere have different temperatures. Which creates rare and denser medium, which in-turn do refraction.
Explanation:
As the star light passing through various layers gets refracted due to temperature(The same reason if we see through something heating and it appears moving), we see them twinkling. Light gets bend at each layer. See the image below:
