I've outlined key areas

Explanation:

Nicotine

Location - The Brain
Damage - Psychological dependence and addiction leading to the alteration of behaviour and mindset

Carbon Monoxide

Location - The Blood
Damage - Enters the blood in the lungs and combines with haemoglobin (the substance responsible for carrying oxygen molecules around the body in the blood), blocking the blood’s ability to carry oxygen to body cells.

Tar

Location - The Lungs
Damage - Resides in the lungs and causes cell alteration and, sometimes, cellular death.

Please note these are the 'main' areas of health concerns for each substance, there are others and they do intact impact on each other. For example, nicotine can 'paralyse' cilia cells in the trachea which further allows more tar to enter the lungs.
