You can use the fact that the slope representes the change in $y$ for a given change in $x$.
Basically:
change in $y$ is $\Delta y = {y}_{2} - {y}_{1}$
${y}_{1} = y$
${y}_{2} = 5$
change in $x$ is $\Delta x = {x}_{2} - {x}_{1}$
${x}_{1} = x$
${x}_{2} = - 3$
$s l o p e = \frac{\Delta y}{\Delta x} = 2$
$2 = \frac{5 - y}{- 3 - x}$
$- 6 - 2 x = 5 - y$
$y = 2 x + 11$
