The Weapons of Mass Destruction that were the primary reason for the invasion were not found. Other bad decisions followed.

Explanation:

The decision to invade Iraq was done with wishful, if not actually manufactured evidence. The invasion was done on the cheap so there was not enough troops on the ground to properly police the country at the end of hostilities and the Iraqi Army was disbanded filling the country with unemployed, trained soldiers.

There were not enough American/British soldiers to guard the huge weapons storage areas in Iraq to prevent them from being looted to arm the newly created sectarian militias. The destruction of old order left a power vacuum that many wanted to seize hold of. The fuse was lit on widespread civil violence.
