What does it mean, if someone has high hydroxyl levels in their blood and how does that result in cardiac arrest?

Blood has a tightly controlled pH of 7.4 due to presence of a blood buffer system, hence Hydroxyl (OH) ion level in blood will be closely monitored by our body.

So an increased level of hydroxyl in blood or it leading to heart attack has got no connection. In fact, there are people who drink alkaline water which is thought to act as an antioxidant for the body but this is a highly debated concept.

There are different types of 'hydroxy' compound drugs , almost all are effective on nervous system and should be taken only under medical supervision: such as hydroxyzine.

Another compound, 5-hydroxytryptophan or 5HTP is of herbal origin and often used to suppress appetite or to treat insomnia but there is lack of scientific research supporting such claims. So it will not be judicious to comment on its side effect.
