Bug report: Answered question appears in the "Open Questions" section without being flagged -"?"

I've found an answered question that appears in the Open Questions section. This usually happens for answered questions that have been flagged for improvement, not for regular answered questions.

As far as I can remember, and Ernest confirmed it as well, no flags were raised for it. This is the first question I see appearing in the OQ section under these circumstances.

Moreover, I don't see who posted it, since it's not listed below the answer.

I see this happen with older questions, but I'm pretty sure this one was actually posted two days ago.

I've also noticed that often when a question indicates that there is 1 answer, when I check it, there is no answer.
