$1.$ Often you add a little to distilled water to your flask during the titration to swill the sides; the presence of a bit of water does not affect the acid/base reagent present.

$2.$ I presume that there was extra $H O A c$ in the flask, the titre (of $N a O H$) will be high.

$3.$ No effect, I would have added 3-4 drops anyway.....because phenolphthalein is colourless at low pH, often you forget you've added indicator already, and give it another dose, to be sure, to be sure.

$4.$ You have not accounted for the extra titrant, concentration will be high.
