The x coordinate is zero so you cannot use $theta = tan^-1(y/x)$ ; you just need to know that this is a special case and the angle is $pi/2$ .

You can still use $r = sqrt(x^2 + y^2)$ but it is easier to visualize that the radius is the same as the y coordinate:

The polar form is $(4, pi/2)$
