You can use the relationship (derived from the definition of slope):
$y - {y}_{0} = m \left(x - {x}_{0}\right)$
where:
$m$ is the slope
(${x}_{0} , {y}_{0}$) are the coordinates of your point.
So:
$y - 12 = \frac{5}{6} \left(x - 22\right)$
Now you can rearrange it.
