I am afraid that both previous answers are wrong, at least partially. The answer is :

So, to understand how to get to this solution, I strongly advise to read the answer wrote by "Anonymous". Just be careful for the last step : 49+25 = 74 and not 64.
Concerning the answer from "9divine9 I.", the first solution is right but I must say that I have no idea how he found the 3 others solutions... which are wrong !
As evidence, look at the following plots :

In green -> the solution (standard form)
N.B : As you can see both are matching perfectly !
In blue : the 2nd solution proposed by "9divine9 I."
And now, the plot of the 3 others solutions proposed by "9divine9 I.", just to compare that none of them match each other.
