I got: #y=1/2x+7/2#

The general reltionship you can use is:

#(y-y_2)/(y_2-y_1)=(x-x_2)/(x_2-x_1)#

for a line passing through points of coordinates #(x_1,y_1)# and #(x_2,y_2)#.

With your values you get:

#(y-5)/(5-4)=(x-3)/(3-1)#

giving:

#y-5=(x-3)/2#

#2y-10=x-3#

#2y=x+7#

in Slope (#m#) and Intercept form (#c#) you have to isolate #y# to get something as:

#y=mx+c#

or in your case:

#y=1/2x+7/2#
