To convert to polar you need the following:

$y = r sin θ$ $y=rsin theta$
$x = r cos θ$ $x=rcos theta$
$r^{2} = x^{2} + y^{2}$ $r^2=x^2+y^2$
$θ = {tan}^{- 1} (\frac{y}{x})$ $theta = tan^-1(y/x)$
