Sticking to basics, these are the main changes of phase changes in matter.

Explanation:

Melting, Evaporation, Condensation and Freezing.

Using water as an example, it freezes at 0 degrees and melts when the temperature is higher. It evaporates at 100 degrees and condenses during a cool day. (Imagine water droplets on your windshield)
