It should be "Who put salt in my coffee?"

Explanation:

Put is used in past tense here, and then there is no extra 's'.
(meaning "Who did put salt ...")

"Who puts salt in my coffee?" may be regarded as a request (!), as the action is still to be done, or at the moment occurring, or that it keeps on happening over and over again, but that's probably not what you mean.
