Could we have the ability to add a comment to a question edit?
Often I find that I need to edit a question in order to make sense of it. Sometimes when there are alternate definitions it would seem appropriate to at least note that the question had been modified and might not reflect the original intention.

I think at this moment there is no such option. In fact once the question is answered we can not even change word/topic.

Explanation:
For starters, you know that you can only edit unanswered questions --once a question is answered, regular contributors can no longer edit its wording or subject/topic.
Now, when you edit an unanswered question, you have the option of leaving a comment in the question description or in the comments section below the question. Granted, these are not quite what you're asking for, but they can get the job done.
For example, you can edit an unanswered question that looks like this
and use the question description to leave a note about your edit
You can add your name if you want, so something like "Edited by Alan P" will give everybody a clear idea of who edited the question.
Alternatively, you can simply leave a comment in the comments section. Something like " I edited the question to show that ..." or "I edited to clarify..." will do the trick.
So, seeing how we can use the current system to leave comments when we edit a question, I think it's safe to say that the team will not tweak the system to add dedicated comments for question edits, at least not for the time being.
